The “Volunteer Evaluation Form Template 2”, created by Volleyball Canada, can be used to evaluate the performance of a volunteer, as well as gain insight from the volunteer on the program.  It provides the volunteer an opportunity to rate the program on its orientation, training and supervision as well as how its volunteer staff structure and/or relationships can be improved.
